Heart Murmur Grades Don’t Always Reflect the Degree of Disease

It is recommend that an echocardiogram be performed in an animal with a heart murmur before anesthesia, if possible. Although listening to the heart for abnormal heart sounds such as murmurs or gallops is an important way to look for heart disease, this only gives you an indication that the animal may have underlying heart disease. It doesn’t necessarily tell you which type of heart disease or how severe it is.

Based on the type of abnormal heart sound, the location and the breed of animal, your veterinarian can often make an educated guess as to what type of heart disease your pet has based on what’s most likely, but this is still not definitive.

Heart murmurs in dogs are typically graded on a scale of 1 through 6, with 1 being the quietest and 6 being the loudest.

In smaller breeds of dogs, the most common cause of a heart murmur is a leak in a heart valve, referred to as chronic valvular disease. With this type of disease, the loudness of the heart murmur typically does correlate to the severity of the disease, though there are rare exceptions to this.

Larger breeds of dogs, however, can commonly get other types of heart disease — such as dilated cardiomyopathy (DCM), a disease that causes the heart muscle to weaken — or even have cancerous tumors grow in the heart. With these diseases, you can have a very quiet murmur and still have severe disease.

The fact that Buddy has a grade 2 heart murmur doesn’t really give us any indication of how serious his heart disease is. He could have very mild disease, where anesthesia wouldn’t be a problem; or he could have more significant disease, where anesthesia may be more worrisome.

An echocardiogram is an ultrasound, or a sonogram of the heart. Much like a sonogram would be performed to look at a fetus, the echocardiogram allows us to visualize the heart, assess the size of the different chambers, evaluate how well the heart is contracting, determine where the blood is flowing within the heart and evaluate the thickness of the heart walls. It even allows us to estimate the pressures inside the heart and inside the lungs.

Using this information, we can diagnose the cause of the abnormal heart sound, how severe the disease is, and what types of risks will be encountered with anesthesia. In many cases, the cardiologist can even provide your veterinarian with specific recommendations for types of medications and fluids to be used during anesthesia that would be safest for the type of heart disease your pet has.

Specifically with dental disease,  treating a pet with certain types of heart disease with antibiotics before the procedure may be recommended to decrease the risk of infections in the heart.

Chicken Dog Treats Made in China Now Linked to 900 Complaints

Nearly 1,000 dogs reportedly have been sickened by chicken jerky pet treats from China, according to a new tally of complaints from worried owners and veterinarians submitted to federal health officials.

The Food and Drug Administration has logged some 900 reports of illnesses and deaths since November, when it warned owners about continued problems with the products known variously as chicken jerky strips, treats and nuggets, a spokeswoman said.

Back then, the agency already had heard from 70 owners about problems ranging from vomiting and diarrhea to kidney failure and other serious ailments after animals reportedly consumed the treats.

Since then, complaints have mounted steadily, putting growing pressure on the FDA to solve the problem.

The agency sent inspectors earlier this year to Chinese plants that make the jerky treats, two Ohio lawmakers previously told msnbc.com. No results of those inspections are yet available, FDA spokeswoman Tamara Ward said Monday.

Despite repeated tests since 2007, FDA scientists have been unable to detect any toxin responsible for the animal illnesses, officials said. The agency has asked certain pet owners to send in samples of suspect treats along with their animals’ veterinary records.

Three top brands of chicken jerky treats were among those most recently cited by pet owners and veterinarians in complaints of harm, FDA records obtained by msnbc.com showed. They included Waggin’ Train and Canyon Creek Ranch brands produced by Nestle Purina PetCare Co., and Milo’s Kitchen Home-style Dog Treats, produced by the Del Monte Corp.

 

Multi-Modal Approach Often Needed for Pet’s Arthritis

Big Jake, a large mixed-breed dog, just turned 8.

 Sean, his owner, has noticed Jake slowing down a bit over the past few months. Jake used to eagerly jump into his spot in the front seat of Sean’s pickup, but now he is much more tentative. There have been times Sean has had to lift Jake’s back end up into the truck, and Sean swears Jake is embarrassed when this occurs. Sean is thinking Jake has developed arthritis and wants to know if there are dietary supplements that might help Jake. Sean has heard about glucosamine potentially helping with arthritis in people and dogs and wanted to know if it might be appropriate for Jake.

Glucosamine is a nutritional supplement thought to have cartilage-protecting properties and is most commonly found paired with chondroitin sulfate. It is used to help alleviate arthritic pain in humans, as well as dogs. The question is, and this is a question commonly raised concerning the use of many supplements touted to treat myriad conditions, does glucosamine really work?

There have been several studies involving humans trying to ascertain whether glucosamine with or without chondroitin sulfate can help people with arthritis. The results have been somewhat equivocal. Some studies report improvement with reduced arthritic pain when taking the supplement, while others have shown no change. That said, glucosamine/ chondroitin sulfate remains a commonly used supplement for arthritis in people, and many patients report improvement with its use. In veterinary medicine, glucosamine/chondroitin has been studied for efficacy in arthritic dogs; the results are similar to studies in humans. Some report it works and others report it worked no better than patients given a placebo. Of course, one must keep in mind that when working with dogs, we cannot simply ask them, “Is your arthritis pain improving?” We instead rely on observation, which can be subjective. Personally, I have used glucosamine/chondroitin sulfate in many of my arthritic patients and have had positive results in some and little or no improvement in others. Long story short: For Jake, I would say it is worth a try. I do, however, recommend some investigation of Jake’s decreased truck leaping ability.

I never prefer to assume when trying to diagnose and then treat patients, and right now with Jake, we are assuming he has arthritis. Indeed, it is a logical assumption, but ultimately, it remains an assumption. I would recommend he see his veterinarian for a skeletal evaluation physically and radiographically.

Arthritis is an inflammatory process involving joints within the body, any or all of them depending on the type of arthritis. The arthritic inflammation does not show up on a radiograph, but what does show are the secondary changes to the bones involved with the arthritic joint. These occur as a result of the inflammation from the arthritis and are a large component of the pain associated with the process.

If Jake does have arthritis, I would recommend he be prescribed a nonsteriodal anti-inflammatory medication (NSAID) along with supplementing with glucosamine/chondroitin sulfate. An NSAID will directly address the inflammatory process within the joint, effectively reducing the reaction that leads to the pain and thus reducing the pain. My dog was on NSAID medication along with glucosamine/ chondroitin sulfate for almost three years. Without them, he would not have had a good quality of life.

Another suggestion I would make concerning supplements to help with arthritis in canines is to use omega 3 fatty acids, specifically those with DHA.

DHA has been shown to reduce inflammation, and I truly believe it is very helpful to these patients. We have available to us chewable treats for dogs that contain these supplements, which make it not only fun for the dog but potentially rewarding for its quality of life. I do wonder, though, if the dog can make the association.

Pets and Pain

Ad special members of our families, it’s difficult to see our cats and dogs suffering. Just like with children, we wish we could take the hurt away. But sometimes that’s just not possible and it’s important to know how to help our furry family members manage pain in uncomfortable situations.

As a surgical specialist at Pet Emergency and Specialty Center of Marin, I have a special passion for helping pets manage painful situations and I urge pet parents to understand a few important points about pets and pain.

1. Understand the different types of pain. Just as in humans, there are two main types of pain your pet can experience — acute pain and chronic pain. Acute pain comes on suddenly and strongly, generally due to injury or trauma. Acute pain typically goes away when the cause of the pain is treated. On the other hand, chronic pain generally lasts longer and develops more slowly. Chronic pain can be difficult to identify and treat because it is often brought on by age or other illnesses.

2. Know how to identify when your pet is in pain. If your pet has just had an operation, it’s safe to say they will be in some pain as they recover. But if the pain source is not so obvious, pet parents should be very aware of changes in behavior. Pets don’t have the ability to complain like we do, so watching for the following behavioral warning signs may help you identify a painful situation:

• Unusually quiet, listless, restless, or unresponsive

* Whining, trembling, whimpering, howling, or constantly meowing

• Biting when they normally wouldn’t

• Constantly licking or chewing at a particular part of the body

• Acting out of character, either aggressively or submissively

• Flattening ears against the head

• Trouble sleeping or eating

• Seeking more affection than usual

• Unable to get comfortable

3. Understand how you can help your pet. PESCM’s veterinary specialists take an individualized approach to pain control. We assess each patient for the source of pain, level of pain, expected duration of pain, along with their personality and any other medical conditions or medications. Understanding that your pet’s needs are unique to it is an important part of managing its pain appropriately.

It’s also important to get involved with your veterinarian in helping to manage the pain because you are often the best judge of your pet’s comfort. I tend to send medications home with a range of doses so clients can vary the amount given at each dose. This empowers them to make the best decisions for their pet as they recover.

4. Know your treatment options and best pain management solutions. Pain control is most effective when used before the pain actually starts. If a pet comes in for a procedure at PESCM, all anesthetic protocols are designed with this in mind. One of the major advantages of having round-the-clock veteerinerian and nursing staff is that patients can be monitored for comfort and kept on the optimal schedule of pain medications after surgery.

It’s also important to point out that many patients will benefit from several different types of pain management therapy. These include multiple classes of pain management drugs, dietary supplements, ice packing or warm compressing, comfortable bedding and even acupuncture.

5. Know what not to do. It’s not OK to give pets human pain medications or even veterinary pain medications for another household pet. Our ER sees too many cases of toxicity and overdose where clients were trying to help their pets be more comfortable by giving them a medication not prescribed to them. There are many options for pain medications in pets with tolerable side effects, but just like in humans, they must be prescribed specifically for your pet to avoid a hazardous situation.

No pet loving family wants to see its pets suffer, so it’s important for all pet parents to be cognizant of their pet’s behaviors. Any time you are in doubt about your pet’s pain level or comfort, it’s always best to consult with your veterinarian.

Keeping Your Pet’s Microchip Information Up-To-Date is Essential

If you want to be proactive about saving your pet’s life, regular veterinary visits, pet insurance and keeping a cat indoors only are certainly high on the list. But the most powerful tool of all could be about the size of a grain of rice: a microchip.

Sometime over the past month or so, HomeAgain, a lost pet recovery service and microchip provider, reunited their one-millionth pet with the owner. (It was a challenge to tell exactly which pet was the one-millionth recovered because so many pets are found through HomeAgain — about 10,000 each month!)

Sadly, one in three family pets will get lost during its lifetime, and without identification, around 90 percent will not return home.

The Baumgardner family, of Lompoc, CA, lost their Pekeapoo, Chewie, two years ago. Ultimately, a microchip made reunification possible, but it took a while.

While the family was living in Arizona, Anita and her husband went out to dinner one evening, leaving Chewie and Jack, a Cocker Spaniel/Labrador mix, at home with the couple’s then 18-year-old son, AJ, and daughter Gaby, 13.

Jack likes to open doors, and as AJ snoozed on the sofa, the pooch slipped out the front door. While Jack strolled only a few feet away to catch some sun, Chewie zipped past him and kept on going.

“When we returned home, we searched the neighborhood, but it was already dark,” says Baumgardner. “We assumed in the morning Chewie would find his way home.” That never happened. The family notified HomeAgain, called local shelters, a local pet store and Chewie’s groomer, all to no avail. Time went by, and eventually the Baumgardners moved to Lompoc, CA.

“We all knew Chewie might have been hit by a car, or who knows what,” says Anita. “The hope was that maybe he was picked up by another family who just didn’t check to see if he had a microchip.”

Having a microchip alone is of little value. It’s like having a cell phone without a phone number. Pet owners need to register their contact information with the microchip provider and keep it up to date. Anita did provide new information when the family moved. In April 2012, she received a call from HomeAgain stating, “We have your dog.”

“I told them, ‘you must be mistaken. My dog is right here,'” Baumgardner recalls, referring to Jack.

“No, it’s Chewie,” said the caller.

“Well, this was two years later. I nearly fell out of my chair,” Anita recalls. It turns out Chewie had been spotted walking along a road and was picked up by a good Samaritan. The pet lover did the right thing, having Chewie scanned for a microchip at a local shelter. Because his registration information was up to date, HomeAgain was easily able to contact Anita.

Family members promptly headed to Arizona to pick up Chewie. Shelter staff said that even before Anita and Chewie were reunited, the dog heard Anita and clearly recognized her voice – even after two years. The reunion was joyous on all sides.

“Chewie looked pretty good. He’d even gained some weight, though he had a few missing teeth,” says Anita.

No one knows exactly where Chewie was for two years; perhaps he’ll write a “tell all” book.

Gaby was especially elated about the reunion. She posted photos every day for weeks on her Facebook page.

“Our dogs are a part of our family, and very important to us,” says Anita. “I think most people feel that way, which is why I’m such an enthusiastic supporter of microchipping.”

Of course, without this service, many of the one million animals recovered through HomeAgain would have been euthanized.

“Our family is sure grateful,” Baumgardner says.

The non-plant concerns in the spring include fertilizers, pesticides, slug bait, mulch, and garden tools. Talk to your local nursery about the safest options, read labels carefully and store everything safely in sealed containers or out of reach.

Try natural products like vinegar for weeds, coffee grounds, beer and salt for slugs, and soap and water as a natural pesticide.

Avoid cocoa mulch as it comes from chocolate manufacturing and can contain substances that will cause minor chocolate poisoning (vomiting, diarrhea, hyperactivity) as well as general irritation to the mouth, stomach and intestines.

Many of our mature dogs (and almost all of our cats) are discriminate — they might sniff but they are not inclined to eat plants.

Grass is often the exception and in small amounts, common grasses are safe. Ornamental grasses can be very irritating to the mouth, throat, and nose so if you have a big grass eater, it is safest to avoid these plants.

Remember that puppies and kittens are always an exception. They will generally eat ANYTHING! It still makes most sense however to always pick the safest plants possible for our spring flower gardens and our deck pots.

Horticulturists employed at our favorite plant nurseries are excellent resources for pet safe plants and gardening products. The ASPCA Animal Poison Control Center has a fantastic guide to pet-safe gardening and a wonderful collection of plant pictures and toxicity information here . PetPlace.com also has an array of informative articles written by veterinarians about toxic plants and gardening.

The three most common spring garden problems we see in our busy Tacoma pet emergency room include dogs ingesting SLUG bait poison (metaldehyde), dogs ingesting decomposing things out of the compost pile, and Lily ingestion or sniffing by cats.

A few bites of slug bait can cause horrible tremors. Quick emergency treatment is critical.

A compost pile snack can also cause tremors or it may cause drunk-like behavior or vomiting and diarrhea. Here too, quick emergency treatment is essential for a quick recovery.

Lilies are highly toxic to cats. It is safest to avoid all lilies — both as cut flowers as part of a bouquet or as a garden plant. Potential sniffing of the flower and inhaling the pollen can even be a problem to our cats.

Enjoy your garden but do your research first. Prevention is so much easier than sick animals and treatment.

Study: Owners Oblivious to Implications of Brachycephalic Syndrome

A recent study by the Royal Veterinary College found that 58% of surveyed owners of dogs with brachycephalic obstructive airway syndrome do not recognize the medical severity of their pet’s problems, such as snoring while awake and difficulty breathing during normal exercise. Instead, owners considered this to be a normal expression of the dog’s breed and not a medical problem that should be addressed by veterinarians. PhysOrg.com

Owners of certain popular “short-nosed” dog breeds, including Pugs, Bulldogs, Pekingese and French Bulldogs, are unknowingly putting their pets’ welfare at risk and not seeking essential treatment because they consider their pets’ signs of breathing difficulties to be ‘normal’ for that breed – so reveals a new study from the Royal Veterinary College, published on Thursday 10 May 2012 by the Universities Federation for Animal Welfare (UFAW) in their journal Animal Welfare. Other affected breeds in the study included the Boston Terrier, Dogue de Bordeaux, Cavalier King Charles Spaniel, and short-nosed crossbreeds.

Cardiac Disease is Prevalent in Pets

                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                       Heart disease is common in dogs and cats and can be an important cause of illness.

Unlike many other health problems, heart disease often profoundly limits a pet’s survival. An important statistic for pet owners to consider is about 10 per cent of dogs that present to veterinarians for care suffer from heart disease. In cats, it has been reported 15 per cent of apparently healthy cats have an underlying heart condition.

What these two statistics clearly tell me is heart disease is prevalent and therefore annual checkups play a vital role in the early identification of heart disease and helping keep your pet healthy.

Heart disease in dogs and cats falls into two categories: congenital and acquired.

Congenital problems manifest at birth and can be due to genetically inherited disorders or arise from problems during pregnancy. Acquired disorders are more common, and usually develop in adulthood or old age.

Recent research in veterinary medicine has identified some inherited disorders that occur later in life in purebred dogs such as the boxer and Doberman pinscher, and in purebred cats such as the Maine coon and ragdoll. In the past several years, genetic tests have been developed to identify these heart disorders and more will be available in the future.

An annual checkup is one of the best opportunities to identify heart disease in dogs and cats.

The annual physical examination includes an assessment of the heart with a stethoscope and evaluation of the peripheral arterial pulse. This examination allows your veterinarian to identify heart murmurs and arrhythmias, and assess if the heart is too weak.

Heart murmurs occur when there are leaks in one of the four heart valves or one of the valves does not open completely. An abnormal communication in the heart can also create a murmur. Arrhythmias are abnormalities of heart rhythm, causing the heart to beat irregularly, or too slow or too fast.

Many forms of heart disease result in a weakened heart, which in turn can lead to heart failure, where the heart cannot pump a sufficient amount of blood through the body. While continuing to work harder to pump blood, further heart damage can occur.

Early detection of these abnormalities through regular checkups helps your veterinarian identify the cause of the heart abnormality and its severity, which goes a long way toward early treatment and offers the best chance for a cure or long survival.

Depending on the nature of your pet’s heart condition, your veterinarian has an array of additional tests to help identify and assess the severity of the heart disease. Chest X-rays are used to evaluate heart size and reveal congestion in the lungs, which is an indicator of a weakened heart.

An electrocardiogram and 24-hour holter exam can assess the heart rhythm. A cardiac ultrasound exam measures the strength of the heart and identifies structural defects. Recent work shows that early treatment with medication can have a significant effect on both survival and quality of life.

As well, many congenital disorders can be treated and cured, but early detection and intervention are key to a good outcome.

The annual checkup is also the best means to help dog and cat breeders identify the healthiest purebred dogs for breeding purposes. This is an important way to limit the number of puppies with congenital and inherited disorders.

In conclusion, annual checkups are an essential way to keep your pet free of clinical signs of both heart disease and other disorders.

MRI on awake dogs measures canine response to humans

Researchers at the Emory University Center for Neuropolicy trained two dogs to stand completely still while inside an MRI scanner and then imaged the dogs’ brains while they responded to human hand signals about treats. The researchers found activity in the caudate area of the brain when the dogs were given a signal indicating they would receive a treat. Additional study could provide insights into the human-canine bond and even human evolution.

How To Keep Dog’s Ears Healthy

I am often asked by pet owners if they need to be cleaning their dog’s ears.  If a dog has normal healthy ears, no cleaning is necessary. But if there are  signs of ear infection, yes, the ears would benefit from a good cleaning with  the appropriate materials.

Signs of ear infections include shaking the head frequently and scratching at  the ears with the back leg. One might also notice a strange smell coming from  the ears. The ear canal can be very red and inflamed. Excessive wax sometimes  accumulates in the ear. This wax could be purulent from infection or thick and  black.

When cleaning the ears, one source of cleaner is your veterinarian’s office.  These cleaners are usually gentle and do not cause undue discomfort. I say  undue, because some discomfort is unavoidable. Even healthy ears do not like  having fluid put in the ear canal. The discomfort is minimized by buffering the  solution’s pH.

Ear cleansers generally have a salicylic or acetic acid base to help break  down the wax and aid in killing yeast. I like the ones that have a good smell,  because it helps to destroy the odor emanating from the ear canal. Ear cleaners  do not have antibiotics in them. They are made to be used frequently and as  needed. If antibiotics were used in this fashion it would compound our problems  with antibiotic resistance.

One mistake I have seen is the use of antibiotic ointment as if it were a  cleaner. Owners put antibiotic drops in the ears once a week or when they see a  flare-up. These medicines were not made to be used this way. Ear washes are to  be used on a daily or weekly basis to prevent infections. Antibiotics are for  treating a severe infection. Occasionally a chronic severe case will warrant  ongoing antibiotic therapy, but these are few and far between.

What can be used at home for washing out the ears? A good natural wash  consists of one part white distilled vinegar to one part rubbing alcohol.  Combine these two ingredients and soak a cotton ball in the mixture. Wipe out  the ear, rubbing the cotton ball so that the alcohol-vinegar mix trickles down  into the ear canal.

The vinegar will kill yeast, and the alcohol will kill bacteria as well as  dry out the ear. The biggest problem with this cleaner is that the alcohol burns  irritated skin. A gentler version would be one part white distilled vinegar to  one part water. It helps treat yeast infections, and it is gentler.

If you find that the ear canal continues to build up with wax even though you  are cleaning out the ear or you see the signs of infection worsening, it is time  to make an appointment with your veterinarian. He can look at cytology and  discover the source of the infection so that a medical treatment can be  prescribed.
